Transplant Compass by ValaBright is a structured digital education model designed to deliver timely, accessible, and personalized transplant-related guidance to patients and care teams across the pre-transplant, perioperative, and post-transplant journey.
Transplant Compass addresses a common challenge in transplant care: patients and caregivers are often asked to understand complex instructions during stressful, time-sensitive, and medically complicated periods. Missed or misunderstood instructions can contribute to delays, cancellations, unnecessary calls, medication errors, avoidable complications, and inconsistent adherence to care plans.
The project uses a structured digital education approach that combines short-form text messaging, accessible web-based reference materials, audio, video, and conditional logic to deliver information at specific points in the transplant journey. Educational content can be organized around key phases such as referral, evaluation, waitlist preparation, pre-operative readiness, discharge planning, medication adherence, symptom monitoring, lifestyle adjustment, and long-term follow-up. The feedback component of the education centers on teach-back for confirming comprehension, as well as health checks that can trigger alerts to transplant staff.
