The Renal Care Pathway: A Collaborative, Proactive Response to the American Kidney Health Initiative

It is essential for patients with End Stage Renal Disease (ESRD) to learn proactive measures to manage their disease and get transplanted. Unfortunately, there are a number of barriers preventing ESRD patients from accessing the appropriate care and education needed for survival. During this discussion, we will review the American Kidney Health Initiative launched in 2019 and explore methodologies employed by transplant centers to improve patient access to transplantation. Join us to hear from Transplant Administrative Director, Edward Strouse, as he discusses the implications of the kidney health initiative and shares how his center rose to the challenge of improving patient care through the “Renal Care Pathway” program.
