Transplant Recipient Registration Form

The form completed and submitted by the transplant center when a patient is transplanted. The form contains patient status, pretransplant clinical measures, transplant procedure, post-transplant clinical measures, graft status, treatment and immunosuppression. (OPTN Glossary May 2021)
