Dr. Agnes Szarek is a pediatric solid organ transplant pharmacist at Ann & Robert H. Lurie Children’s Hospital of Chicago. Agnes collaborates with multidisciplinary teams for liver, kidney, intestinal and heart transplants. She provides comprehensive medication management from the pre-transplant evaluation and perioperative management to long-term post-transplant follow up. She educates patients and caregivers on transplant medications to promote adherence. Beyond patient care, Agnes is involved in quality improvement initiatives and contributes to the education and training of pharmacy practice residents.
The Lifelong Challenge of Adherence in Pediatric Transplantation

During this session we will explore the challenges of transitioning pediatric patients—especially those with developmental delays or adherence issues—to adult care. A major component of the program will examine the challenges of nonadherence in pediatric transplantation, discussing the impact that nonadherence has on patient outcomes and the overall success of the transplant process. Strategies to promote lifelong adherence and support patients’ psychological well-being throughout the transition process will also be discussed, with a focus on effective education and mental health support for both patients and families.
