Maciej Bielaczyc

Mike Bielaczyc, LCSW, CCTSW earned his Master’s Degree in Social Work at Jane Adams College of Social Work at the University of Illinois in Chicago in 2011. Prior to joining Northwestern Medicine, he provided services to individuals with serious, chronic mental illness as an intake coordinator and psychosocial rehabilitation case worker. He has been a social worker at Northwestern Memorial Hospital since 2011, providing discharge planning to General Medicine patients until 2015, before joining the lung transplant team. He is currently a Certified Clinical Transplant Social Worker and provides psychosocial assessments and services to pre-transplant and posttransplant lung transplant patients and their families
