Brief Bio

Debby McGrath is a social worker for the Division of Pulmonary medicine at The Children’s Hospital of Philadelphia, with a focus on Interstitial Lung Disease, Cystic Fibrosis and Lung Transplant. She has worked in the division since 2002. Debby leads the Cystic Fibrosis and Lung Transplant Transition Program which coordinates semiannual meetings with Hospital of University of PA Adult Lung Center to discuss medical and psychosocial information of soon to be transitioned patients. She has been a member of the Division of Pulmonary Medicine Transition Team since 2015 which developed best practice policies for transition. This committee advises and provides recommendations to the division for a successful patient transition. She has also been a member of the Cystic Fibrosis Inpatient Transition to Adult Center Team that works to address patients and families concerns when transitioning care teams. Finally, she was a co-leader in the CF Foundation Learning & Leadership Collaboration. The work from this collaboration was the foundation for the current CFF guidelines for CF Lung Transplant referral and timing.

From Pediatrics to Adulthood: A Multidisciplinary Approach to Transitioning Transplant Care

There are several critical aspects related to the successful transition of pediatric solid organ transplant recipients to adult care, requiring a collaborative effort from the multi-disciplinary team involved. During this discussion, we will review a specific case study that illustrates how challenging the transition can be and demonstrates how each role – including pediatric psychology, social work, and medicine – works together to move patients and families through the transition process.
